Roster Night Checklist

o Copies of two public tryout announcements

o List of all players who tried out for your team this season including birthdates and phone numbers

o Registration Form with Coaches’ Signatures (must be typed)

o Completed Roster including player signatures and birth certificate number for all players (must be typed)

o New Player sign up sheet (list of players that require original birth certificates)

o Coaches Code of Conduct signed by all coaches

o Background Check forms for all coaches

o Two copies of completed roster

o Email Vice President PDF file of completed roster (Vice President will forward all PDFs to all teams)

For each player that is double-rostered

o A completed dual participation form

For any player that is waivered

o A completed waiver form

For returning players that you have both the white and yellow copy of their last year’s contract

o White copy of last year’s contract

o Yellow copy of last year’s contract

o New contract for 2012 (both copies)

For returning players that you DO NOT have both the white and yellow copy of their last year’s contract

o New contract for 2012 (both copies)

o Original Birth Certificate

For new players

o New contract for 2012 (both copies)

o Original Birth Certificate

Please note: All players names on rosters MUST appear on rosters as seen on their Birth Certificates. NO nicknames.
